Job Summary: Direct Service Providers (DSPs) are responsible for supporting the health, safety, and well-being of residents by following individualized care plans, assisting with daily living activities, providing toileting and meal assistance, supporting medication self-administration, monitoring health changes, and maintaining a safe, respectful environment. This role requires flexibility, compassion, and a commitment to high-quality, person-centered care.

The Community Activities Leader will provide oversight for Emerald's Adult Day Services in the delivery of quality of care to seniors and adults with physical and mental disabilities in the communities. The Community Activities Leader will ensure members’ needs are met in terms of personal care, safety and execution of all other program.Community Activities Leader provides transportation and personal care for senior adults & adults with disabilities on community outings, excursions, to and from the Adult Day Program.
